Special Needs

PrimeCare Pediatrics serves as a Medical Home for many special needs children.  This approach provides pediatric care that facilitates partnerships between patients, clinicians, medical staff, and families.  In a Medical Home:

• Patients have a relationship with a personal physician who functions as member of a care team that takes collective responsibility for the patient's ongoing care.

• Primecare Pediatrics own dedicated special needs nurse is able to coordinate services for patients within the community while ensuring that all the patients needs are met.

• The care team is responsible for providing and arranging all the patient's health care needs.

• Patients can expect care that is coordinated across care settings and disciplines.

• Patients experience enhanced access and communication.
